    Just place it in dir (D:\AVEVA\Plant\PDMS12.0.SP6\PMLLIB\), run command 'pml rehash all' and than U may have a possibility to see Your form by typing 'show !!name_of_your_from'.

    I suggest to create your own PMLLIB folder instead default PMLLIB

    You may also add a toolbar button to run the show command : 'show !!name_of_your_from'
