Try to reload the form by command: PML RELOAD FORM !!APHREVISIONMARK
Command to show the form: SHOW !!APHREVISIONMARK
I have notice that you are using PDMS 11.6, I may not sure that all the macro code is compatible with the version you are using. Just try then post the error if you will encounter it.
I am not sure if it will work with E3D, I have seen in other post that cloudmark is already available in E3D. We do not have E3D to test if it does work on it.
